Pharmacodynamics and pharmacokinetics
The pharmacological group of Cordiamin is an analeptic , that is, it has a stimulating effect on the central nervous system, and, mainly, on such centers in the medulla oblongata as the respiratory and vasomotor centers. The reflex effect also partially excites the respiratory center due to the excitation of chemoreceptors on the carotid glomeruli. Breathing under the influence of the drug Cordiamin becomes faster and deeper (especially in a depressed state), while in the case of initial hypotension , blood pressure rises.
The drug is able to facilitate the transmission of impulses between nerve cells - neurons , which increases excitability, as well as the sensitivity of the centers to natural irritants, for example to carbon dioxide .
Cordiamine does not have a direct stimulating effect on the heart or a vasoconstrictor effect. It is known to be toxic in large doses and can cause clonic seizures .
Regarding pharmacokinetics, good absorption regardless of the route of administration. The effect of the injection occurs after a few minutes, from oral administration - after 12-15 minutes. The duration of the effect varies depending on the type of administration:
- intravenously - 1-1.5 hours;
- intramuscularly – 2-3 hours;
- orally – within 3-4 hours.
In the liver, the drug is converted into inactive metabolites and is excreted primarily by the kidneys.
Composition and dosage form
Cordiamine contains the main active substance - niketamide, which has the ability to quickly dissolve in water purified from foreign impurities in the form of metals. The active ingredient is added to all dosage forms of the drug at a concentration of 25%. 1 ml of medication contains 250 mg of niketamide, with no additional components in the composition. Thanks to its unique chemical structure, this remedy is able to penetrate into the center of the medulla oblongata, affecting its activity.
Today, dosage forms of Cordiamin are presented in the following variations:
- Drops. Intended for internal use, available in glass bottles of 15, 25 and 30 ml. The bottles are equipped with special nozzles, making it easy to dose the liquid.
- Solution for internal use. It is produced in a bottle similar to drops, but the dosage forms have different names due to the release of drugs by different pharmacological companies. Therefore, the drops and the solution are identical in their structure and mechanism of action.
- Injection solution. Available in the form of ampoules containing 1 and 2 ml of active substance. The form is intended for injection administration (intravenously, intramuscularly, subcutaneously). The dosage depends on the expected effect and the patient's condition.
Indications for use of Cordiamine
The drug is prescribed to weaken the respiratory function of the lungs in patients with infectious diseases , in particular during the period of convalescence (recovery), in case of poisoning with narcotic, hypnotic or analgesic drugs, as well as in hypotensive states .
Indications for the use of Cordiamin as an additional remedy are collapse , fainting , shock of various etiologies, asphyxia , including in newborns.
Pharmacological properties
Under the influence of the medication, the transmission of nerve impulses to neurons is normalized, as a result of which the sensitivity and excitability of the respiratory center to the effects of various stimuli increases. Taking Cordiamin helps suppress the process of inhibition in the brain and activate impulses for a long time.
As a result of increased activity of the vasomotor center, peripheral vascular resistance increases, which increases blood pressure.
Side effects
The following reactions are possible:
- state of anxiety;
- muscle twitching, starting with the orbicularis oris muscle;
- convulsions are usually clonic in nature; with increasing dose, clonic-tonic convulsions are possible;
- redness of the face;
- allergic reactions , itching or hives ;
- nausea and vomiting ;
- various heart rhythm .
Instructions for use of Cordiamin (Method and dosage)
Cordiamine is taken orally (tablets and drops taken regardless of meals) or parenterally (injections). For Cordiamine drops, the instructions for use suggest a dosage for adults - 15-40 drops at a time, 2 or 3 times a day, for children - the number of drops per dose should correspond to the age of the child.
The solution is administered subcutaneously, intramuscularly or intravenously (parenterally). For adults, the dose is 1-2 ml 1 to 3 times a day, for children (recommended subcutaneously) - from 0.1 ml to 0.75 ml, depending on age.
The maximum permissible single dose for adults is 2 ml (corresponding to 60 drops), the daily dose is 6 ml (corresponding to 180 drops). The maximum single dose of subcutaneous and intravenous administration for drug poisoning is 5 ml.
Features of reception at low pressure
Cordiamine at low pressure allows you to increase blood pressure in a short time. After taking the dosage forms of the drug, the nerve endings are irritated, which leads to the activation of vital processes in the body. The patient experiences a narrowing of the vascular lumen, resulting in an increase in blood pressure.
Taking Cordiamine is indicated for patients with increased arterial tone. The narrowing of the circulatory system occurs under the influence of certain structural elements of the brain, so the patient experiences a sharp increase in blood pressure. - sleeping pills, narcotics, antiepileptics, anxiolytics and antipsychotic drugs - reduces their effect;
- Reserpine enhances the convulsive effect of Cordiamine.
Contraindications and side effects
The use of the drug is contraindicated in the presence of the following conditions, which in case of overdose can also be detected as side effects:
- allergic reaction to the active substance;
- predisposition to epileptic and convulsive seizures;
- hypertension;
- rapid heartbeat with normal blood pressure;
- hyperthermia;
- pregnancy period.
Important! While using Cordiamin, you should stop taking ascorbic acid, as they are incompatible. The medication enhances the effect of antidepressants and stimulants and reduces the therapeutic effect of analgesics, barbiturates, and tranquilizers.
