The composition of the drug includes the active substance - sucralfate - a disaccharide, including aluminum hydroxide and sucrose sulfate. When taking the drug, rapid healing of peptic ulcers , prevention of the development of gastric and duodenal , and protection of the gastric mucous membranes from the influence of pepsin and hydrochloric acid .

Treatment with Venter can reduce pepsin by 30%, without having any particular effect on the acidity of gastric juice. In this case, the mechanism of action of the drug is based on the binding of sucralfate to the proteins of the affected tissues of the mucous membranes, causing the formation of a protective film on the surface of the ulcer. The drug accelerates the regeneration of damage, scarring of ulcers, prevents the development of relapses of ulcers and the formation of stress ulcers . The absorption of phosphates in the gastrointestinal tract also decreases.

When taken orally, the substance is slightly absorbed into the systemic circulation. In the body, the drug does not form clinically significant concentrations in plasma and does not exhibit a systemic effect. The substance is not metabolized inside the body. Partial excretion of the drug occurs through the kidneys, the rest - unchanged through the intestines.

Description of pharmacological action

Gastroprotective agent. By interacting with the proteins of necrotic ulcer tissue, it forms a protective layer that prevents the further destructive effect of pepsin, hydrochloric acid and bile salts. Increases prostaglandin synthesis, reduces pepsin activity and binds bile salts. Inhibits pepsin activity by 30%. Has a weak antacid effect. Being in the stomach and duodenum, it is transformed into a polyanion with many free negative charges, which form strong bonds with positive radicals of protein-like exudate (albumin, fibrinogen) of the mucous membrane. Interaction with unchanged mucous membrane is insignificant.

Pharmacokinetics

Absorption is low - 3-5% of the administered dose (up to 5% of the disaccharide component and less than 0.02% of aluminum). Excreted through the intestines - 90% unchanged; a small amount of sulfate disaccharide that enters the bloodstream is excreted by the kidneys.

Use with caution during pregnancy and lactation.

Contraindications for use

Severe renal dysfunction, dysphagia or gastrointestinal obstruction, gastrointestinal bleeding, hypersensitivity to sucralfate.

Side effects

Directions for use and doses

Orally for adults, 0.5-1 g 4 times a day. or 1 g morning and evening, or 2 g 2 times a day. 1 hour before meals and before bedtime. The maximum daily dose is 12 g. The average duration of treatment for peptic ulcer is 4-6 weeks, if necessary - up to 12 weeks. Children - 0.5 g 4 times a day. In patients with hyperphosphatemia, when plasma phosphate concentrations decrease, the dose of sucralfate may be reduced.

Interactions with other drugs

When used simultaneously with indirect anticoagulants, their anticoagulant activity (including warfarin) may decrease. With simultaneous use, the absorption of antimicrobial agents of fluoroquinolone derivatives is reduced. With simultaneous use, the absorption of amitriptyline is reduced, which may lead to a decrease in its clinical effectiveness. It is believed that with simultaneous use, the formation of chelate complexes with amphotericin B sucralfate, colistin sulfate, and tobramycin is possible, which can lead to a decrease in their antimicrobial activity. When used simultaneously with digoxin, the absorption of digoxin may be reduced. It is believed that with simultaneous use there may be a slight decrease in the absorption of ketoconazole and fluconazole. When used simultaneously with levothyroxine, the effectiveness of levothyroxine is noticeably reduced. When used concomitantly with sucralfate, slight changes in the pharmacokinetics of theophylline were observed. It is also believed that there may be a significant reduction in the absorption of theophylline from sustained release dosage forms. It is believed that with simultaneous use, a decrease in the absorption of tetracycline is possible. With simultaneous use, the absorption of phenytoin and sulpiride decreases. A case has been described of a decrease in the concentration of quinidine in the blood plasma when used simultaneously with sucralfate. With simultaneous use, a slight decrease in the bioavailability of cimetidine, ranitidine, and roxatidine cannot be ruled out.

Special instructions for use

If simultaneous use of antacids is necessary, they should be taken 30 minutes before or 30 minutes after taking sucralfate. In case of renal failure, monitoring of serum aluminum and phosphate concentrations is necessary. The appearance of drowsiness and convulsions may indicate manifestations of the toxic effects of aluminum. Short-term treatment can lead to complete scarring of the ulcer, but does not change the frequency and severity of relapses of peptic ulcer disease after scarring. Venter granules and tablets, instructions for use (Method and dosage)

According to the instructions for use of Venter, the drug in tablets or granules is taken orally. In this case, the tablets are swallowed whole with the required amount of water. When the whole tablet is difficult to swallow, it can be dissolved in water and then drunk. The granules must be pre-dissolved in water.

The prepared solution should be taken 30-60 minutes before meals. The duration of therapy is determined by a specialist, taking into account the complexity of the disorder and the individual characteristics of each patient.

For adult patients, the daily dosage is 2 tablets or 2 sachets - one taken before lunch, the second before bed.

If the patient is diagnosed with hyperphosphatemia , then the dosage is reduced. The presence of peptic ulcers allows the daily dose to be increased by 2 times. The therapeutic course for patients with peptic ulcers of the stomach and duodenum is about 12 weeks. The disappearance of symptoms is not a signal to discontinue the drug; as a rule, treatment is continued until a diagnostic examination that confirms scarring of the ulcers.

Prevention of stress ulcers is carried out by prescribing for adult patients one tablet or a bag of granules to be taken 6 times a day. To prevent relapse of peptic ulcers, take one tablet or sachet 2 times a day. The maximum permissible daily dosage is 8 tablets or sachets. In case of renal dysfunction, patients have a reduced therapeutic course.

Children over 4 years old are recommended to take 0.5-1 tablet no more than 4 times a day. It is not recommended to take the drug in the form of granules in childhood.

Special instructions for the use of the drug Venter

The small amount of aluminum contained in sucralfate is absorbed in the digestive tract and subsequently excreted in the urine. In case of severe renal dysfunction, aluminum may accumulate in the body with the development of its toxic effect. The risk of accumulation increases with simultaneous administration of other drugs containing aluminum (for example, some antacids). The safety and effectiveness of sucralfate in children under 4 years of age have not been sufficiently studied, so it is not recommended to prescribe the drug to children in this age group. The drug can be used during pregnancy only when the expected benefit to the expectant mother outweighs the potential risk to the fetus. There is no evidence that sucralfate passes into breast milk, so it should be used with caution during breastfeeding.

Overdose

Combination therapy can reduce the absorption of tetracyclines , cimetidine, ciprofloxacin, ranitidine, norfloxacin, digoxin , indirect anticoagulants, ofloxacin and theophylline . In this case, it is also necessary to maintain an interval that should be more than 2 hours.
