Pharmacodynamics and pharmacokinetics
The active component of the drug, St. John's wort extract , prevents the development of neurotransmitter transmission disorders, modulates the secretion of interleukin-6 and inhibits catechol-O-methyltransferase and monoamine oxidase. At the same time, manifestations of emotional lability, depressed mood, anxiety and tension disappear.
There are no data available on the pharmacokinetics of Gelarium Hypericum.
Pharmacological properties of the drug Gelarium hypericum
Antidepressant of plant origin. A standardized extract from the herb St. John's wort prevents the development of neurotransmitter transmission disorders (norepinephrine, serotonin), inhibits MAO and catechol-O-methyltransferase, modulates the secretion of interleukin-6, which determines the mechanism of its antidepressant and anxiolytic action (eliminates feelings of anxiety, tension, improves mood). Due to the presence of the drug, in addition to its own antidepressant effect, adaptogenic and anxiolytic properties, its use is advisable for complex (comorbid) anxiety-depressive conditions. The drug is indicated for neurotic depression. After oral administration, the maximum concentration of active substances in the blood plasma is achieved after 5 hours for hypericin and 3 hours for pseudohypericin. Compared to pseudohypericin, hypericin enters the blood with some delay, which is obviously due to their different lipophilic properties. The half-life of hypericin and pseudohypericin has a high level of variability and is 10–37 and 6–30 hours, respectively.
Contraindications for use
It is not recommended to take Gelarium Hypericum if:
- individual hypersensitivity to it;
- children under 12 years of ;
- lactase deficiency, lactose and fructose intolerance, glucose-galactose malabsorption, sucrase or isomaltase deficiency;
- the presence of photosensitivity of the skin to UV radiation;
- simultaneous use with Cyclosporine , Tacrolimus , indinavir , amprenavir and other HIV protease inhibitors, for example, in the treatment of HIV-positive patients, as well as with warfarin, irinotecan and MAO inhibitors. In such cases, it is necessary to maintain an interval of at least 14 days;

Interaction
Simultaneous use of Gelarium Hypericum with indirect anticoagulants - phenprocoumon, warfarin, imatinib and other cytostatics, midazolam, digoxin, amitriptyline, fexofenadine, benzodiazepine, nortriptyline, theophylline, methadone, verapamil, simvastatin and finasteride can significantly reduce their effectiveness.
Combination therapy with antidepressants , for example: nefazodone, paroxetine, sertraline, Buspirone, triptans, often leads to an increase in their effect, as well as the development of undesirable symptoms: nausea, vomiting, fear, anxiety, confusion .
Sometimes, when taken together, Gelarium Hypericum enhances the photosensitizing effect of a number of drugs: tetracyclines, thiazide diuretics, sulfonamides, quinolones and piroxicam.
Also, this drug may prolong the sleep caused by general anesthesia or narcotic analgesics, but shortens the sleep caused by barbiturates. A decrease in the hypotensive effect of reserpine and a decrease in the concentration of cyclosporine and indinavir in the blood were noted.
Concomitant use with desogestrel and ethinyl estradiol increases the risk of bleeding and metabolic rate. of digoxin in the blood and its effectiveness decrease
oral contraceptives are taken during treatment , then patients need to take care of additional methods of contraception.
Drug interactions Gelarium hypericum
Simultaneous use with a number of drugs can lead to a decrease in their effectiveness: anticoagulants of coumarin derivatives (phenprocoumon, warfarin), indinavir, verapamil, cyclosporine, theophylline, digoxin. Simultaneous use with oral hormonal contraceptives in some cases may be accompanied by intermenstrual bleeding, and with other photosensitizing drugs - an increase in their effect. Concomitant use with antidepressants - MAO inhibitors can enhance the serotonergic effect, so there should be an interval of at least 14 days between taking these drugs.
