Lumbago: treatment of lumbago with NANOPLAST forte medical patch

The term "lumbago" is used to describe sudden sharp pain (lumbago) in the lower back. This is a vertebrogenic syndrome, which is characterized by pronounced pain symptoms, decreased range of motion, lumbar deformities and muscle tension. Simply put, lumbago is a back pain in which the patient cannot straighten up due to severe pain in the lower back. Most often it affects men over 30 years of age. Lumbago, the symptoms and treatment of which will be discussed in this article, is a common reason for visiting the CELT pain clinic. We help our patients get rid of pain symptoms and return to a normal lifestyle. Patients often come to us with lumbago of the neck or lumbago in the thoracic region, for whom we provide professional medical care!

Causes

Lumbago can occur for various reasons:

  • damage to the intervertebral disc due to heavy physical exertion;
  • displacement of the vertebrae due to osteochondrosis;
  • congenital anomalies of the vertebrae and, as a consequence, splitting of the vertebral arches, abnormal shape of the intervertebral joints, fusion of the 1st–3rd vertebrae, abnormal shape of the vertebrae, spondylolisthesis.

Rare causes of lumbago include:

  • benign and malignant neoplasms that destroy the vertebra;
  • diseases of an infectious nature leading to the development of inflammatory processes in the spine;
  • rheumatic diseases.

What is lumbago with sciatica

Lumbago with sciatica is a pathological condition in which lower back pain is combined with inflammation or pinching of the sciatic nerve. In this case, painful sensations are localized not only in the lumbar region, but also spread to the sacrum, to the buttock, to the femoral area and further down the leg.

The combination of lumbago and sciatica most often occurs due to displacement of the intervertebral discs. It can also signal severe disorders in the spine caused by serious diseases (malignant tumors, purulent processes, etc.).

Clinical manifestations

Most often, men aged 30 to 40 years are affected by lumbago. Painful symptoms manifest themselves due to irritation of receptors that are located in the area of the fibrous ring of the affected disc or ligaments that are adjacent to it. As a result, tonic muscle tension occurs.

The occurrence of lumbago is most often observed during or after intense physical activity and, as a rule, can be triggered by excessive overheating, which is accompanied by cooling. The pain that occurs in this case has a tearing, shooting, pulsating character. It seems to the patient that they are localized deep in the bone and muscle tissues. Any attempt to make a movement leads to increased pain and the development of muscle spasm. The attack may last only a few minutes and disappear as suddenly as it appeared, or it may last for several days. Pain sensations are reduced when lying on a hard surface and at rest.

Symptoms of lumbago

Many patients with back pain improve so quickly that extensive differential diagnosis and research are not necessary.

However, back pain can be prolonged or recur very often. Depending on the duration of symptoms, lumbago can be classified as "acute" or "chronic":

  • acute lumbago - duration less than six weeks;
  • subacute lumbago - from six weeks to three months;
  • chronic lumbago - lasts longer than three months.

Diagnostics

Before prescribing treatment for lumbago, specialists at the CELT clinic conduct a diagnostic study, which allows you to correctly diagnose and identify the cause of the lumbago.

During the examination, the doctor pays attention to the following:

  • assessing the condition of the lower back muscles;
  • determining the nature of pain and its intensity;
  • checking motor function and sensitivity;
  • determining the nature of the lesion through a series of tests.

In addition to being examined by a doctor and taking a medical history, you may need to:

  • comprehensive neurological examination;
  • radiography of the spine;
  • CT scan;
  • Magnetic resonance imaging;
  • spinal scintigraphy;
  • electroneuromyography.

One of the problems in diagnosing this pathology is the need for it to be carried out simultaneously along with pain relief and muscle spasm relief.

Establishing diagnosis

The main task of the specialist is to distinguish lumbago from sciatica, kidney dysfunction and radiculitis. The syndrome is diagnosed based on examination of the patient and characteristic complaints.

Additional research methods include:

  • X-ray of the spine (with and without contrast) and nuclear magnetic resonance. Determines the presence of tumors, intervertebral hernia and osteoporosis.
  • CT scan. Makes it possible to visualize the condition of the soft tissues surrounding the spine.
  • Electromyoneurography. Registers electrical impulses in muscles and nerve roots, which makes it possible to identify innervation disorders.
  • General blood and urine analysis. Eliminates the possibility of inflammation.

Monotherapy ESWT for lumbago: review of scientific literature

N.I. Bodnya and colleagues observed 3050 patients aged 35-80 years with myofascial pain syndrome, of which 23.3% (715 people) suffered from osteochondrosis of the lumbar spine. They received a course of treatment, from 3 to 7 procedures, a total number of impulses of 5000 beats per session with a frequency of 14-20 Hz with low amplitude vibration, affecting triggers and adjacent muscles. In 75%, the pain syndrome regressed at the end of treatment, with a prolonged effect for six months. The motor deficit also decreased quite quickly [4].

HER. Ryzhova and co-authors used ESWT in the treatment of 47 patients with myofascial syndrome, of which 26 people suffered from lumbodynia (VAS pain – 8 points). The main group included 15 people, and 9 - the control group. Against the background of standard pharmacological treatment, shockwave therapy was used in the main group, and SMT, phonophoresis, magnetic therapy, and electrophoresis were used in the control group. The device generated radial (R-SW applicator - 1500-2000 shock waves per trigger point) and focused acoustic waves with the F-SW applicator from 100 to 500 shock waves per point, course of up to 8 procedures [16]. In the main group, the pain syndrome was relieved: from 0 to 2 points on VAS for 87% of patients; people moved more actively, and the duration of the VUT did not exceed a week. In the control group, the proportion of people with a positive effect was half as much, and the duration of temporary disability was 10-12 days [16].

More details

The standard treatment regimen for lumbago includes the following:

  • Kyphosis of the spinal column - placing the patient in bed on his back with his legs bent at the pelvis and knees, under which any object is placed that allows them to be kept in this position. This approach allows you to get rid of pain and ensures quick recovery;
  • Drug treatment - taking Aspirin (3-4 times a day, 0.25 g - 1 g), Ibuprofen (3-4 times a day, 0.2-0.8 g), Diclofenac (2 –3 times a day, 0.025–0.05 g) to eliminate inflammatory processes and relieve pain symptoms. According to the doctor's indications, it is possible to take sedatives and sleeping pills. To eliminate muscle spasms, Spazmolitin is used, the dosage of which is selected individually.

Bed rest must be observed for 7 to 10 days, during which the above medications are used. If pain symptoms are very pronounced, a novocaine blockade is performed. It is important to eat right and exclude from your diet foods that retain water in the body. Treatment is comprehensive and, in addition to the above, includes:

  • manual and vacuum therapy;
  • acupuncture;
  • electrical stimulation;
  • professional massage;
  • laser therapy.

Patients of the CELT multidisciplinary clinic do not tolerate pain! Our doctors do everything in their power to eliminate pain symptoms as quickly as possible, since a prolonged pain attack negatively affects the patient’s condition.

Treatment of lumbago at the MART clinic

Treatment of lumbago is always based on an objective diagnosis. For any etiology of pain in a particular patient, the principle of an integrated approach between the efforts of the patient and doctors works:

  • The patient should pay close attention to his lifestyle, especially nutrition and physical activity and the recommendations of doctors;
  • The specialists of our clinic will help relieve acute pain that occurs in the lumbar region, improve the condition of the musculo-ligamentous apparatus of the spine, restore the functions of the spine as much as possible, and obtain a sustainable therapeutic effect.

Drug treatment

Pain reduction occurs through the use of NSAIDs (non-steroidal anti-inflammatory drugs) in topical gel or capsule form, which reduce inflammation and soothe pain.

Muscle relaxants are used in the treatment of paravertebral muscle spasm, which significantly reduces pain. By taking muscle relaxants, we increase the clinical effectiveness of concomitant NSAIDs. The latter have an analgesic effect when administered in low doses, and have an anti-inflammatory effect when administered in higher doses.

For acute pain, glucocorticosteroids may be prescribed.

Physiotherapeutic treatment

For the treatment of back pain at the MART clinic there is a large selection of modern physiotherapeutic products and equipment:

  • electrophoresis,
  • laser phoresis,
  • magnetophoresis,
  • ultraphonophoresis,
  • peloidotherapy (mud therapy),
  • vacuum therapy.

Manual therapy

In the first four to six weeks, manual therapy may help, the goal of which is to relieve muscle spasms and make joints more flexible. As a result, the pain slowly disappears.

However, the hands of a professional chiropractor alone will not help with pain, although many people think that massage can make the pain go away on its own. If you do not maintain your own activity, with chronic lumbago the pain will quickly return.

Prevention

It is well known that problems with the spine most often arise due to weakening of the body’s muscular activity due to a sedentary lifestyle. That is why the best prevention of lumbago is an active lifestyle, regular swimming, fitness, frequent walking or cycling. By following simple rules, you can keep your spine in good condition and avoid the risk of developing diseases, including lumbago:

  • It is necessary to properly organize the workplace and purchase a chair with an anatomically shaped back and armrests;
  • It is important to properly adjust the seat in the car, and use a massage pad on long trips;
  • When lifting weights, you need to correctly distribute the load: first bend your knees, straighten your back, and only then stand up, loading your legs;
  • When carrying weights, you need to distribute them evenly on both hands;
  • It is best to sleep on your side, on a pillow that exactly matches your shoulder height;
  • Hypothermia must be avoided.

What is the difference between lumbago and lumbodynia?

Lumbago, in fact, is a variant of lumbodynia, which is a general term for all long-term pain in the lumbosacral area. It can have different causes, related and unrelated to spinal diseases. Lower back pain can appear due to diseases of the internal organs (kidneys, gall bladder, liver, uterus, ovaries and others). Also, its occurrence is facilitated by disruption of the innervation and blood supply to the spinal column. It often accompanies pregnancy.

The importance of physical therapy

Gymnastic exercises at the first stage of treatment can relieve spasms. Improving blood circulation in the problem area helps to cope with tissue swelling and prevent the occurrence of congestion.

The gradual restoration of motor activity helps to distribute the load evenly throughout the spine. This approach is considered one of the most effective methods of preventing the appearance of new protrusions or the growth of old ones. A set of exercises is selected individually to provide the patient with sufficient, but not excessive, load, taking into account the severity of his condition and the characteristics of the pathology.

Exercise therapy in combination with manual therapy allows you to achieve the highest possible results in the treatment of lumbago, relieving the syndrome within a few days. To prevent recurrence of the syndrome, chiropractors and orthopedists recommend not abandoning physical therapy after completing the course of therapy. For clients of our center there is a modern exercise therapy room, where activities are carried out to get rid of lumbago and prevent its recurrence.

Diet

During the treatment of lumbago, the patient needs to follow a diet. The diet is based on the following products:

  • lean beef or chicken;
  • low-fat fish;
  • low-fat dairy products;
  • various cereals;
  • vegetables and fresh herbs;
  • fruits, berries, natural juices.

You should completely avoid fatty and irritating foods: smoked meats, pickles, marinades, spices and herbs, fatty meat or lard. Flour and sweet foods can be consumed in moderation. Avoid alcohol completely.

Diagnosis and treatment of lumbar lumbago

Making a correct diagnosis is one of the important points. To do this, the neurologist must conduct an in-depth and targeted history taking and conduct a clinical and neurological examination. It is necessary to clarify with a patient with lumbago the causes and circumstances of the pain, its nature, localization and irradiation, as well as the presence of accompanying symptoms. Physical examination includes inspection, palpation, active movement and sensation.45

When treating patients with lumbago in the lower back, the main task is to provide optimal conditions for rehabilitation, eliminate pain, prevent its chronicity and prevent exacerbations. Bed rest is recommended for 1-3 days. But at the same time, it is important to return to your usual activity as soon as possible in order to prevent the disease from becoming chronic. Previously, treatment for lumbago included prolonged bed rest, but scientists have found that this tactic contributes to chronic pain.45

Typically, local non-steroidal anti-inflammatory drugs (NSAIDs) are used to relieve pain. They suppress the active inflammatory process and help eliminate pain. Additional measures for the treatment of nonspecific lumbar pain can be muscle relaxants and vitamin complexes. They allow you to potentiate the effect of NSAIDs and achieve rapid pain regression.44, 45

First aid for an attack

An attack of lumbago requires medical attention. To alleviate a person’s condition while waiting for a specialist, it is necessary:

  • take a horizontal position, place your legs bent at the knees on an elevation so that the angle between the hips and pelvis is 90 degrees;
  • avoid sudden movements;
  • take tablet analgesics (ibuprofen, etc.), apply anti-inflammatory ointment to the lumbar area.
  • If possible, additionally secure the lumbar region with a corset or tightly wound fabric.

Further treatment should be carried out only under the supervision of a physician. This applies not only to taking medications, but also to gymnastics, as well as massage.
