The active components of the drug are corpuscular components and metabolic products of Escherichia coli and hydrocortisone. 1 g of ointment contains in 166.7 mg of an aqueous suspension 500 million E. coli (corpuscular components and metabolic products) and 3.3 mg of liquid phenol (corresponding to 3.0 mg of pure phenol) as a preservative and 2.5 mg hydrocortisone. Excipients: lanolin, petroleum jelly.
Description
A homogeneous ointment from yellowish to slightly beige in color with a characteristic phenol odor.
Pharmacological properties
The drug stimulates the processes of local immunity due to antigenic action, causing both nonspecific and specific immune reactions. Accelerates and improves wound healing, has an anti-inflammatory effect, prevents reinfections and superinfections.
Indications for use
Local treatment of symptoms of hemorrhoids, eczema and anal fissures.
Contraindications
Hypersensitivity to the active substance or any component of the drug. Specific skin conditions (tuberculosis, syphilis, gonorrhea) in the area of application, chicken pox, reaction to vaccination, fungal diseases, inflammatory diseases of the facial skin (oral dermatitis, rosacea), other bacterial, viral, parasitic infections in the area of application, ulcerative lesions and atrophic changes at the site of application, tumors of the anorectal area.
Use during pregnancy and lactation
Glucocorticosteroids cross the placenta. To date, there is no clear data on the development of teratogenic effects in humans, similar to those observed in animal experiments. Effects on the fetus or newborn (uterine growth retardation, adrenocortical suppression) have been reported with systemic use of large doses of corticosteroids. Although little information is available regarding the use of corticosteroid ointments during pregnancy, given their limited systemic absorption, it is possible to use weak to moderate (Class 1 and Class 2) corticosteroids, such as hydrocortisone, for short periods of time and on limited areas of the skin. . The above effects cannot be excluded with prolonged use or application to large areas of skin. This can only be done if there are special indications. During breastfeeding, it can only be used for short-term treatment and application to small areas. For long-term treatment or application to large areas of affected skin, breastfeeding should be discontinued.
Directions for use and doses
Before use, it is necessary to carry out hygiene procedures for the anal area. For adults, apply the ointment to the affected area of skin in the morning and evening, preferably after bowel movements, gently rubbing in with your finger. The duration of the course of treatment is determined by the doctor depending on the nature and course of the disease. Usually the period of use is 5-7 days.
Side effect
In some cases, patients with hypersensitivity to the components of Posterisan Forte ointment may experience allergic skin reactions. Phenol used as a preservative may cause local skin irritation. With long-term use of the drug in high doses, the following dermatological reactions are possible: dermatitis, eczema, contact dermatitis, delayed healing of wounds and cracks, development of secondary infection, atrophy of the mucous membrane, adrenocortical suppression. May cause skin atrophy, steroid acne, dilation of capillaries (telangiectasia), and the appearance of stretch marks on the skin.
Overdose
No cases of overdose or intoxication have been reported. An overdose causes suppression of the function of the hypothalamic-pituitary-adrenal system. In case of accidental oral use of the ointment (for example, by children), this may lead to gastrointestinal upset (abdominal pain, nausea).
Interaction with other drugs
When using Posterizan Forte ointment simultaneously with other drugs containing corticosteroids (in the form of tablets, drops or injection solutions), the therapeutic and side effects of glucocorticosteroids may be enhanced.
Precautionary measures
Treatment is continued until symptoms disappear, but not longer than 5-7 days. When treated with the drug due to the auxiliary substance Vaseline, with the simultaneous use of condoms, it can lead to a decrease in tensile strength and, as a result, to a decrease in the reliability of such condoms. A combined drug containing two active components - a suspension of a bacterial culture containing inactivated bacteria and their metabolic products, and hydrocortisone. Stimulates the T-system of immunity, increases the phagocytic activity of leukocytes and cells of the reticuloendothelial system, increases the role of a number of humoral factors of nonspecific immunity (induces the formation of cytokines), enhances the formation of antibodies at the site of exposure. Concentrated lipopolysaccharides of cell membranes and metabolites of E. coli of various strains, incl. resistant to antibiotics and other antibacterial agents, lead to an increase in local tissue resistance to the effects of pathogenic microflora. Hydrocortisone (contained in a small dose) complements the effect of a bacterial culture suspension due to its anti-inflammatory, antiallergic and antipruritic effect, which is manifested in reducing exudation, normalizing the permeability and tone of blood vessels, reducing swelling, hyperemia, itching, and stimulating the regeneration of damaged tissues.
Directions for use and doses
Externally, rectally. Apply a thin layer to the affected area in the morning and evening, as well as after each bowel movement. For deeper introduction of the ointment into the anal canal, you can use a screw-on applicator. Rectal suppositories are used in the acute period of the disease, 2–3 pcs. per day. After eliminating the subjective symptoms of the disease, the affected area continues to be lubricated once a day or 1 supp. 1 time per day for several days. If the drug is used to treat chronic dermatitis, then after the disappearance of skin manifestations, it is recommended to extend therapy for another 1–2 weeks. The combined use of ointments and suppositories is possible. After completing the course of treatment, to consolidate the therapeutic effect, it is advisable to use a drug (ointment or suppositories) that does not contain hydrocortisone for another 2-3 weeks.
