Trimedat: how to take and contraindications

The drug enhances peristalsis and promotes gastric emptying, reduces pressure on the esophageal sphincter, acts throughout the intestine, enhances the response of the smooth muscle tissue of the large intestine to various food irritants.

For diseases of the digestive tract associated with impaired motility, trimebutine helps restore the physiological activity of the intestine.

The medicine was initially registered in 2007 under the name Trimedat.

and was produced by a Russian company from a foreign-made finished dosage form (Dae Han New Pharm Co. Ltd., Korea).

In 2014, the drug was registered in Russia by Valenta Pharmaceuticals OJSC as Trimedat Valenta.

. At the moment, all stages of production, including the production of the finished dosage form, are Russian.

Like Trimedat, Trimedat Valenta is available in the form of white tablets containing the active ingredient trimebutine maleate 100 mg

There are some differences in the appearance of the tablets. The Korean tablets were marked with embossed alphabetic and geometric symbols, different for 100 and 200 mg dosages. Domestic-produced Trimedat is made in the form of round flat-cylindrical tablets with a chamfer and two intersecting marks on one side for a dosage of 100 mg, with a chamfer and a score on one side for a dosage of 200 mg.

The composition of the excipients included in the Trimedat and Trimedat Valenta tablets also differs. In addition to lactose monohydrate, starch (corn and potato, respectively), colloidal silicon dioxide, magnesium stearate and talc, Russian-made tablets include povidone (improves the dissolution and bioavailability of medicinal substances).

Indications for use of Trimedat

In the instructions for the drug Trimedat Valenta
is recommended for the treatment of irritable bowel syndrome and postoperative paralytic ileus.

In the instructions for the previous version of the drug, which received permission for distribution in the Russian Federation ( Trimedat

), in addition to the purposes mentioned above, the following are indicated:

  • motor disorders in functional diseases of the gastrointestinal tract
  • gastroesophageal reflux disease (GERD)
  • dyspeptic disorders in gastroduodenal diseases (abdominal pain, digestive disorders, nausea, vomiting)
  • preparation for x-ray and endoscopic examinations of the gastrointestinal tract

The report by O. N. Minushkin, made at the conference “Practical Gastroenterology 2015,” emphasized that the prescription of drugs for the treatment of functional disorders of the gastrointestinal tract, such as Trimedat, should be monitored by instrumental examination methods. In particular, peripheral electrogastroenterography makes it possible to determine the motility of the stomach, small and large intestines before prescribing a drug, during administration and after the end of drug therapy. The slides from the report presented below show that after treatment with Trimedat, the electrical activity of the jejunum and duodenum is leveled out, motor activity improves and the rhythmicity coefficient of the colon increases.

Trimedat and gastrointestinal motility

Trimebutine regulates intestinal motility by influencing its enkephalinergic system.
The drug has no effect on the central nervous system. Being an agonist of peripheral opiate receptors of various types (δ-, μ- and k-), located on neurons of the enteric nervous system and on smooth muscle cells throughout the gastrointestinal tract, it has a stimulating effect in hypokinetic conditions of the latter and acts as an antispasmodic in case of its excessive tone . Trimedat acts throughout the gastrointestinal tract. The figures below show changes in gastrointestinal motility parameters during treatment of GERD in children with Trimedat. To assess the nature of gastrointestinal motility, the method of peripheral electrogastroenterography (PEGEG) was used (Akobyan A.N., Belmer S.V., Vykhristyuk O.F. et al.). Trimebutine normalizes visceral sensitivity and thus has an analgesic effect in abdominal pain syndrome.

Trimebutin affects the humoral regulation of gastrointestinal motility, increasing the secretion of motilin and reducing the levels of gastrin, glucagon, pancreatic polypeptide, insulin and vasoactive intestinal peptide (Akopyan A.N., Belmer S.V., Vykhristyuk O.F. et al. with reference to Delvaux M., Wingate D.

Procedure for use and dosage of the drug Trimedat

Trimedat tablets are taken orally.
Adults and children over 12 years of age are prescribed 100-200 mg 3 times a day. For the treatment of irritable bowel syndrome during an exacerbation, 600 mg of Trimedat per day is prescribed for 4 weeks. To prevent relapse of the disease, further administration of the drug at a dose of 300 mg per day for 12 weeks is recommended.

For children 3-5 years old, Trimedat is prescribed 25 mg 3 times a day, for children 5-12 years old - 50 mg 3 times a day.

Contraindications to the use of Trimedat

Hypersensitivity to trimebutine maleate and other components included in the drug.
Lactase deficiency, lactose intolerance, glucose-galactose malabsorption.

Children under 3 years of age - for dosage forms of Trimedat Valenta; children under 12 years of age - for dosage forms of Trimedat.

Pregnancy.

Use of Trimedat during pregnancy and lactation

Experimental studies have not revealed data on the teratogenicity and embryotoxicity of Trimedat. However, due to the lack of necessary clinical data, the use of the drug during pregnancy is contraindicated. It is also not recommended to prescribe Trimedat during lactation due to the lack of reliable clinical data confirming the safety of its use during this period. If it is necessary to use Trimedat during lactation, breastfeeding should be stopped.

Side effects of Trimedat

During treatment with the drug, the following side effects may occur:

  • from the digestive system - dry mouth, unpleasant taste, diarrhea, dyspepsia, nausea, constipation
  • from the nervous system - drowsiness, fatigue, dizziness, headache, anxiety
  • allergic reactions - skin rash
  • others - urinary retention, in women - menstrual irregularities, painful enlargement of the mammary glands

The use of the drug in pediatric practice makes it possible to successfully relieve dyspeptic disorders in gastrointestinal motility disorders.

How to take Trimedat: before or after meals

The drug begins to work an hour after it enters the gastrointestinal tract. To prevent food intake from causing pain, belching, bloating and other unpleasant symptoms, Trimedat should be taken 20-30 minutes before the intended meal. When the food is processed by gastric juice and enters the duodenum, the drug will already begin to act and will not allow the food to stagnate, which will reduce the risk of developing rotting processes, which cause flatulence, nausea and belching.

Instructions for Trimedat (Method and dosage)

How to take: orally, without chewing and with water. Instructions for use of Trimedat recommend a standard treatment regimen: 100-200 mg three times a day. The maximum dose is 600 mg per day.

In order to prevent relapse of irritable bowel syndrome, the drug is prescribed at 300 mg per day. The medicine can be taken for 4 months.

Instructions for the use of tablets in pediatric practice

25 mg three times a day for children 3-5 years old. Children 5-12 years old are prescribed 50 mg three times a day. How to take Trimedat: before or after meals? Taking medication is not associated with food intake.

How to take Trimedat for constipation

The causes of constipation and its nature determine the duration of medication use. In case of one-time stool retention due to poor nutrition and low mobility, it is enough to take the drug three times a day in a standard dosage until the intestines can empty. Most often this takes a day, less often – up to 5 days. In this case, the absence of relapse can be guaranteed by the patient himself by changing eating habits and adjusting daily physical activity.

In case of constipation that occurs due to intestinal obstruction caused by circulatory disorders in the inferior vena cava, Trimedat is taken over a long course, starting from 4 weeks. To avoid relapse and worsen the situation, the patient is prescribed a diet high in fiber-rich plant products. This approach to nutrition will ensure softness of the stool, which will significantly facilitate its passage through the intestines, and will eliminate cases of injury to the walls by hard lumps of feces.
