Compound

One tablet of Polyoxidonium contains 12 mg of the active substance azoximer bromide , as well as potato starch (Amylum solani), lactose monohydrate (Lactose monohydrate), stearic acid (Acidum stearicum) as auxiliary components.
One bottle of lyophilisate contains 6 mg of active ingredient, as well as mannitol (E421 Mannit), povidone (Povidonum) and beta-carotene (Betacarotenum) as auxiliary components.

To make one rectal-vaginal suppository, 6 or 12 mg of active ingredient and auxiliary components are used: mannitol (E421 Mannit), povidone (Povidonum), beta-carotene (Betacarotenum), cocoa butter (Butyrum Cacao).

Release form

Polyoxidonium has three dosage forms:

  • pills;
  • rectal-vaginal suppositories;
  • lyophilized powder.

The suppositories are torpedo-shaped, light yellow in color and have a specific smell of cocoa butter.

The tablets are flat-cylindrical, each of them has a chamfer. Their color can vary from yellowish-white to orange-yellow. The presence of slightly noticeable particles of more intense color is allowed in tablets.

Pharmacodynamics and pharmacokinetics

Polyoxidonium (or azoximer bromide ) helps increase the body's resistance to infections caused by viruses, bacteria or fungi.

The mechanism of its action is associated with the substance’s ability to influence phagocytic cells and NK cells (natural or natural killer cells), as well as stimulate the process of antibody formation.

The drug helps restore the immune status in secondary immunodeficiency conditions, caused by various types of infections, injuries, burns, malignant tumors , complications after surgical interventions, treatment with chemotherapy drugs, including cytostatic agents and steroid hormones .

Along with the immunocorrective effect of azoximer, bromide also has a pronounced detoxifying effect, which is due to the structure and high-molecular nature of this substance.

The result of its effect on the body is an increase in the resistance of cell membranes to the cytotoxic (causing damage to cells up to their death) action of drugs and chemicals, as well as a decrease in the toxicity of the latter.

Prescribing Polyoxidonium in combination with other drugs significantly increases the effectiveness of therapy, shortens its duration, allows you to reduce doses or completely avoid the use of antibiotics , bronchodilators and glucocorticosteroids , and helps to increase the period of remission (that is, the period of weakening or complete disappearance of the symptoms of the disease).

Polyoxidonium is well tolerated by patients, does not exhibit mitogenic or polyclonal activity, does not have antigenic properties, does not provoke the development of allergies , mutations and other defects in fetal development , does not have a teratogenic effect on the developing fetus, and has no carcinogenic or embryotoxic properties.

Azoximer bromide is characterized by a high bioavailability, which is about 89 percent. When administered intramuscularly, Cmax in blood plasma is observed 40 minutes after injection.

The substance is distributed fairly quickly in tissues. In the body , azoximer bromide is biotransformed. Its elimination is carried out mainly by the kidneys.

Pharmachologic effect.

Polyoxidonium® has an immunomodulatory effect, increases the body's resistance to local and generalized infections. The basis of the mechanism of immunomodulatory action of the drug Polyoxidonium® is a direct effect on phagocytic cells and natural killer cells, as well as stimulation of antibody formation.

Polyoxidonium® restores immunity in secondary immunodeficiency conditions caused by various infections, injuries, burns, autoimmune diseases, malignant neoplasms, complications after surgery, the use of chemotherapeutic agents, cytostatics, steroid hormones.

Along with the immunomodulatory effect, Polyoxidonium® has pronounced detoxification and antioxidant activity, has the ability to remove toxins and heavy metal salts from the body, and inhibits lipid peroxidation. These properties are determined by the structure and high-molecular nature of the drug Polyoxidonium®. Its inclusion in the complex therapy of cancer patients reduces intoxication during chemotherapy and radiation therapy, and in most cases allows for standard therapy without changing the regimen due to the development of infectious complications and side effects (myelosuppression, vomiting, diarrhea, cystitis, colitis and others).

The use of the drug Polyoxidonium® against the background of secondary immunodeficiency states can increase the effectiveness and shorten the duration of treatment, significantly reduce the use of antibiotics, bronchodilators, glucocorticosteroids, and extend the period of remission.

The drug is well tolerated, does not have mitogenic, polyclonal activity, antigenic properties, does not have allergenic, mutagenic, embryotoxic, teratogenic and carcinogenic effects.

Pharmacokinetics . When administered intramuscularly, Polyoxidonium® has high bioavailability (89%); time to reach maximum concentration in the blood - 40 minutes; quickly distributed throughout all organs and tissues. The half-life of distribution in the body (fast phase) is 0.44 hours, the half-life (slow phase) is 36.2 hours. In the body, the drug is hydrolyzed to oligomers, which are excreted primarily by the kidneys.

Indications for use.

Correction of immunity in adults and children from 6 months.

In adults in complex therapy:

· chronic recurrent infectious and inflammatory diseases that are not amenable to standard therapy in the acute stage and in remission;

· acute and chronic viral and bacterial infections (including urogenital infectious and inflammatory diseases);

· tuberculosis;

· acute and chronic allergic diseases (including hay fever, bronchial asthma, atopic dermatitis), complicated by chronic recurrent bacterial and viral infections;

· in oncology during and after chemotherapy and radiation therapy to reduce the immunosuppressive, nephro- and hepatotoxic effects of drugs;

· to activate regenerative processes (fractures, burns, trophic ulcers);

· rheumatoid arthritis, long-term treatment with immunosuppressants; with rheumatoid arthritis complicated by acute respiratory infections;

· for the prevention of postoperative infectious complications;

· for the prevention of influenza and acute respiratory infections

In children in complex therapy:

· acute and chronic inflammatory diseases caused by pathogens of bacterial, viral, fungal infections (including ENT organs - sinusitis, rhinitis, adenoiditis, hypertrophy of the pharyngeal tonsil, ARVI);

Methods of using the drug Polyoxidonium®: parenteral, intranasal. Methods of application are selected by the doctor depending on the severity of the disease and the age of the patient.

Intramuscularly or intravenously (drip): the drug is prescribed to adults in doses of 6-12 mg 1 time per day every day, every other day, or 1-2 times a week, depending on the diagnosis and severity of the disease .

For intramuscular administration, the contents of the ampoule or bottle are dissolved in 1.5-2 ml of 0.9% sodium chloride solution or water for injection. For intravenous (drip) administration, the drug is dissolved in 2 ml of 0.9% sodium chloride solution, Hemodez-N, Reopoliglucin or 5% dextrose solution, then sterilely transferred into a bottle with the indicated solutions of 200-400 ml.

The prepared solution for parenteral administration cannot be stored.

Intranasal: a dose of 6 mg is dissolved in 1 ml (20 drops) of distilled water, 0.9% sodium chloride solution or boiled water at room temperature.

Recommended treatment regimens for adults . Parenteral:

For acute inflammatory diseases

:
6 mg daily for 3 days, then every other day with a total course of 5-10 injections.
For chronic inflammatory diseases:

6 mg every other day for 5 injections, then 2 times a week for a course of at least 10 injections.

For tuberculosis:

6-12 mg 2 times a week for a course of 10-20 injections.

In patients with acute and chronic urogenital diseases:

6 mg every other day for a course of 10 injections in combination with chemotherapy.

For chronic recurrent herpes:

6 mg every other day for a course of 10 injections in combination with antiviral drugs, interferons and/or inducers of interferon synthesis.

For the treatment of complicated forms of allergic diseases

: 6 mg, course of 5 injections: the first two injections daily, then every other day. For acute allergic and toxic-allergic conditions, administer 6-12 mg intravenously in combination with antiallergic drugs.

For rheumatoid arthritis:

6 mg every other day for 5 injections, then 2 times a week for a course of at least 10 injections.

In cancer patients:

before and during chemotherapy to reduce the immunosuppressive, hepato- and nephrotoxic effects of chemotherapeutic agents, 6-12 mg every other day for a course of at least 10 injections; further, the frequency of administration is determined by the doctor depending on the tolerability and duration of chemotherapy and radiation therapy; for the prevention of the immunosuppressive effect of the tumor, for the correction of immunodeficiency after chemotherapy and radiation therapy, after surgical removal of the tumor, long-term use of the drug Polyoxidonium® is indicated (from 2-3 months to 1 year) at a dose of 6-12 mg 1-2 times a week.

In patients with acute renal failure, it is prescribed no more than 2 times a week.

Methods of using the drug Polyoxidonium®: parenterally, intranasally, sublingually. Methods of application are selected by the doctor depending on the diagnosis, severity of the disease, age and body weight of the patient.

Parenterally, the drug is prescribed to children from 6 months at a dose of 3 mg (intramuscular or intravenous drip 0.1-0.15 mg/kg) daily, every other day or 2 times a week with a course of 5-10 injections (dose calculation is indicated in the table).

For intramuscular administration, the drug is dissolved in 1 ml of water for injection or 0.9% sodium chloride solution.

For intravenous drip administration, the drug is dissolved in 1.5-2 ml of a sterile 0.9% solution of sodium chloride, Reopoliglucin, Hemodez-N or 5% dextrose solution, sterilely transferred to a bottle with the indicated solutions with a volume of 150-250 ml.

Intranasal and sublingual: daily at a daily dose of 0.15 mg/kg for 5-10 days. The drug is administered 1-3 drops into one nasal passage or under the tongue after 2-3 hours .

To prepare a solution for intranasal and sublingual use, a dose of 3 mg is dissolved in 1 ml (20 drops), a dose of 6 mg is dissolved in 2 ml of distilled water, 0.9% sodium chloride solution or boiled water at room temperature. One drop of the prepared solution (50 μl) contains 0.15 mg of Polyoxidonium®, which is prescribed per 1 kg of child’s body weight.

Store the solution for sublingual and intranasal use in the refrigerator for no more than 7 days. Before use, the pipette with the solution must be warmed to room temperature (20-25 o C).

Recommended treatment regimens for children.

For acute inflammatory diseases

: 0.1 mg/kg every other day for a course of 5-7 injections.

For chronic inflammatory diseases

: 0.15 mg/kg 2 times a week for a course of up to 10 injections.

For acute allergic and toxic-allergic conditions:

Indications for use of tablets

For adult patients and adolescents over 12 years of age, Polyoxidonium tablets are prescribed, as a rule, as an addition to the current therapy.

The medicine is indicated for acute and chronic recurrent inflammatory and infectious diseases in the oral cavity, paranasal sinuses, upper respiratory tract, inner and middle ear.

For immunocompromised patients (patients with secondary immune deficiency ), Polyoxidonium is prescribed to be taken sublingually as a monotherapeutic agent for the prevention of influenza and other acute respiratory infections in the period preceding seasonal epidemics.

Indications for use of lyophilisate

The solution, which is prepared from lyophilized powder, is intended for injection into a muscle or vein, as well as for external use.

For adult patients it is prescribed as an addition to the main therapy for:

  • inflammatory diseases of various origins resistant to standard therapy during their exacerbation and during remission (including chronic and recurrent diseases);
  • rheumatoid arthritis (if the patient has undergone long-term treatment with immunosuppressive drugs );
  • complicated by acute respiratory infections or acute respiratory viral infections, rheumatoid arthritis ;
  • infectious diseases occurring in acute or chronic form, provoked by the activity of viruses or bacteria (including diseases of the urogenital tract);
  • tuberculosis;
  • diseases of an allergic nature, which are complicated by chronic recurrent bacterial or viral infection;
  • conditions arising during radiation or chemotherapy of malignant neoplasms.

As an addition to the main therapy, Polyoxidonium is used in cases where it is necessary to reduce the hepato- and nephrotoxic effects of drugs . Since the drug stimulates tissue regeneration, its use is also advisable for the treatment of burns , bone fractures and trophic ulcers .

Monotherapy using Polyoxidonium injections is indicated:

  • to prevent the development of infectious complications in the postoperative period;
  • for the correction of secondary immunodeficiency , which are a consequence of the aging of the body or the adverse effects of external factors;
  • for preventive purposes to prevent influenza and acute respiratory infections .

Children can use Polyoxidonium in the form of a solution from six months. The drug is prescribed as an addition to the main therapy:

  • for infectious and inflammatory diseases (including ENT diseases - adenoid vegetations , ARVI , inflammation of the pharyngeal tonsils, nasal mucosa or paranasal sinuses, etc.);
  • for acute allergic reactions and toxic-allergic lesions;
  • bronchial asthma occurring with complications ;
  • atopic dermatitis complicated by purulent infection ;
  • in conditions accompanied by disruption of the normal bacterial composition of the intestine;
  • for the rehabilitation of children with CBD syndrome ;
  • for preventive purposes to prevent influenza and acute respiratory infections .

Indications for use of suppositories

Indications for the drug in the form of suppositories for rectal and vaginal use are the same as for the solution. The use of suppositories is advisable for inflammatory diseases of various localizations and various etiologies, arthritis , for the treatment of tumors , for the prevention of infectious diseases in children and the elderly.

Polyoxidonium suppositories are often prescribed for the treatment of female diseases. Suppositories in gynecology are prescribed for cervical erosion , endometritis , leukoplakia , colpitis , salpingitis , dysplasia , etc.

add 1-2 ml of 0.9% sodium chloride solution or water for injection to the contents of the bottle and leave for 2-3 minutes to swell, then mix the contents with rotational movements.

For IV drip administration

  • for the prevention of influenza and acute respiratory infections.
  • Prescribe 3 drops in each nasal passage every 1-2 hours (at least 3 times a day) for 5-10 days.

For IV drip

administration, the drug Polyoxidonium® is dissolved in 1.5-2 ml of sterile 0.9% sodium chloride solution. After adding the solvent, the drug is left for 3-5 minutes to swell, then mixed with rotational movements. The dose calculated for the patient is sterilely transferred into a bottle/bag with 0.9% sodium chloride solution with a volume of 25-50-100-200 ml.

The prepared solution for parenteral administration cannot be stored.

Intranasal and sublingual:

Patients with impaired renal function

Instructions for Polyoxidonium (Method and dosage)

The dose and route of administration will be determined depending on the diagnosis, the characteristics of the course of the disease and the age of the patient.

Instructions for Polyoxidonium tablets

The tablets are intended for sublingual (under the tongue) or oral (inside) administration. They are drunk daily, once, twice or three times a day (the frequency of doses depends on the nature of the disease and the severity of the clinical picture) 20-30 minutes before meals.

A single dose of the drug for adults is 1 tablet (equivalent to 12 mg of azoximer bromide ), children from 3 to 10 years old are prescribed to take 0.5 tablets per dose.

In cancer patients:

- before and during chemotherapy to reduce the immunosuppressive, hepato- and nephrotoxic effects of chemotherapy drugs - 6-12 mg every other day for a course of at least 10 injections, then the frequency of administration is determined by the doctor depending on the tolerability and duration of chemotherapy and radiation therapy;

— for the prevention of the immunosuppressive effect of the tumor, for the correction of immunodeficiency after chemotherapy and radiation therapy, after surgical removal of the tumor, long-term use of the drug Polyoxidonium® is indicated (from 2-3 months to 1 year) - 6-12 mg 1-2 times a week.

In patients with acute renal failure:

6 mg/day is prescribed for the treatment of acute and chronic infections of the ENT organs, to enhance the regenerative processes of the mucous membranes, to prevent complications and relapses of diseases, to prevent influenza and acute respiratory infections. 3 drops in each nasal passage every 2–3 hours (3 times a day) for 5–10 days.

Parenterally

the drug is prescribed to children from 6 months at a dose of 3 mg (im or intravenous drip 0.1–0.15 mg/kg) daily, every other day or 2 times a week in a course of 5–10 injections (dose calculation is indicated in the table ).

For intramuscular administration, the drug is dissolved in 1 ml of water for injection or 0.9% sodium chloride solution.

For intravenous drip administration, the drug is dissolved in 1.5–2 ml of a sterile 0.9% solution of sodium chloride, Reopoliglucin, Hemodez-N or 5% dextrose solution, sterilely transferred to a bottle with the indicated solutions with a volume of 150–250 ml.

Intranasal and sublingual:

daily at a daily dose of 0.15 mg/kg for 5–10 days. The drug is administered 1–3 drops into one nasal passage or under the tongue after 2–3 hours. To prepare a solution for intranasal and sublingual use, a dose of 3 mg is dissolved in 1 ml (20 drops), a dose of 6 mg is dissolved in 2 ml of distilled water, 0.9% sodium chloride solution or boiled water at room temperature. One drop of the prepared solution (50 μl) contains 0.15 mg of the drug Polyoxidonium®, which is prescribed per 1 kg of child’s body weight.

Store the solution for sublingual and intranasal use in the refrigerator for no more than 7 days. Before use, the pipette containing the solution must be warmed to room temperature (20–25 °C).

Recommended treatment regimens for children:

- for acute inflammatory diseases:

0.1 mg/kg every other day for a course of 5–7 injections;

- for chronic inflammatory diseases:

0.15 mg/kg 2 times a week for a course of up to 10 injections;

- for acute allergic and toxic-allergic conditions:

IV drip at a dose of 0.15 mg/kg in combination with antiallergic drugs;

— for the treatment of complicated forms of allergic diseases in combination with basic therapy:
