Causes of balanoposthitis in boys
In boys, balanoposthitis most often occurs before the age of five. The reason is physiological phimosis, which is normal for boys under 3-5 years of age, but makes spontaneous cleansing of the head difficult due to the fact that the foreskin over it is narrowed and cannot be completely exposed. As a result, a yellowish-white preputial secretion—smegma—accumulates under the foreskin, and microbes that cause inflammation actively develop in it. Sometimes parents try to open the head of the child’s penis themselves, injuring the foreskin and causing infection.
