What is oligohydramnios in pregnancy (medical term oligohydramnios)?
This is a pathological condition in which the amount of amniotic fluid decreases (less than 500 ml) while the membranes are intact and without signs of water leakage. Amniotic fluid (synonymous with amniotic fluid) surrounds the fetus and is the medium that connects it and the mother’s body. Their main function is to ensure the vital activity of the fetus, nutrition and its development, protection from infections and injuries, and facilitating the movements of the developing fetus. Amniotic fluid appears in the third week and normally its volume changes with the duration of pregnancy: at 10 weeks it is 30 ml, then gradually increases and by 37-38 weeks it reaches 1500 ml, and by the end of the period it decreases again. Typically, at the end of pregnancy, moderate oligohydramnios is physiological and does not pose a danger. Oligohydramnios in the later stages, at 30, 32, 38, 40 weeks

Pathogenesis

The volume of liquid is affected by:

  • Fetal urination is the main source in the 2nd half of pregnancy, if there are kidney abnormalities, very little amniotic fluid is formed. In a normal full-term pregnancy, the fetus produces 700-900 ml of urine per day.
  • Ingestion of water by the fetus. At full term, he swallows 230-750 ml per day.
  • Secretion of fluid by the lungs.

When IUGR is detected in the third trimester, conservative therapy is carried out, but it is effective only in 12% of cases (with an AF index of at least 3 cm and a gestational age of up to 36 weeks). In the presence of a urogenital infection, antibacterial therapy is indicated.

Severe oligohydramnios, developing in the second trimester, IUGR lead to termination of pregnancy and antenatal fetal death. If congenital malformations of the fetus are detected that are incompatible with life, termination of pregnancy is indicated for medical reasons.

Childbirth in women with oligohydramnios occurs with complications. The most common symptoms are weakness of labor and fetal hypoxia. Due to compression of the umbilical cord, intrapartum fetal death may occur. In the afterbirth and early postpartum periods, a high incidence of bleeding is observed. Childbirth is carried out using early amniotomy (at the beginning of the first stage of labor) under constant cardiac monitoring of the fetal condition. Prevention of bleeding in the afterbirth and early postpartum period is carried out (methylergometrine, oxytocin). If fetal hypoxia develops during labor, emergency CS delivery is indicated. The incidence of oligohydramnios ranges from 0.7% to 5.5% . The diagnosis of oligohydramnios is based on determining the amount of amniotic fluid using ultrasound and calculating the amniotic fluid index (AFI).

The amount of amniotic fluid is considered normal if the AFI value is in the range from 5 to 8 cm , an AFI of 2 to 5 cm is regarded as moderate oligohydramnios, and an AFI of less than 2 cm is severe oligohydramnios.

The appearance of oligohydramnios does not depend on the age of the patients (the average age during the studies was 26 years). It occurs with equal frequency in both primiparous and multiparous women. In approximately 50% of patients with oligohydramnios, the disease is combined with the threat of miscarriage. A history of infectious and inflammatory diseases in pregnant women with moderate oligohydramnios occurred in up to 30% of cases.

With oligohydramnios, premature birth occurs 2 times more often than during the normal course of pregnancy, and this happens more often with severe oligohydramnios. During full-term pregnancy, the most common complication of labor due to oligohydramnios is weakness of labor (up to 80% of births).

With severe oligohydramnios, acute fetal hypoxia occurs more often than during normal childbirth (about 20%). With severe changes, intrauterine fetal death is possible. Therefore, the percentage of delivery by cesarean section is high (up to 40 - 50%).

With moderate oligohydramnios, quite often (about 20%) children are born with hypotrophy of varying severity . But at the same time, the outcome of pregnancy is favorable for the vast majority of women - children are born in satisfactory condition.

In newborns born from mothers with severe oligohydramnios, signs of hypotrophy of varying degrees are more often observed (up to 75%). Children are sometimes born with asphyxia, with aspiration of meconium amniotic fluid.

An ultrasound examination makes it possible to clarify the severity of oligohydramnios and intrauterine growth retardation. Doppler study of blood flow is carried out in the umbilical cord artery, in the middle cerebral artery of the fetus and both uterine arteries of pregnant women. As the amount of amniotic fluid decreases, the frequency of pronounced forms of IGR (grades 2 and 3) increases. Sometimes, with severe oligohydramnios, premature “ripening” of the placenta is detected. In 70% of pregnant women with oligohydramnios, Doppler blood flow measurements show changes in hemodynamic parameters. To clarify the condition of the fetus with oligohydramnios, cardiotocography is required after 32 weeks of gestation.

Pregnant women with oligohydramnios are prescribed complex therapy aimed at improving uteroplacental blood flow, metabolism in the placenta, restoring gas exchange in the mother-placenta-fetus system and treating the underlying disease. During treatment, it is advisable to conduct an ultrasound every 7 - 10 days, Dopplerography of vessels in the mother-placenta-fetus system once every three days, and daily CMN.

Against the background of pathogenetic therapy, in patients with IUGR of 1 and 2 degrees at 34-35 weeks of gestation, an increase in the amount of amniotic fluid is observed, which is accompanied by an increase in body weight in the fetuses and an improvement in their hemodynamic parameters. This allows you to prolong pregnancy as much as possible. Up to 34 weeks of pregnancy, blood flow disturbances in the mother-placenta-fetus system can be functional, and this gives hope for their improvement and normalization of the fetal condition with adequate therapy.

If the fetal condition deteriorates according to CTG data and indicators of fetal-placental blood flow, delivery is carried out by emergency cesarean section.

At the same time, in pregnant women with severe oligohydramnios and the absence of amniotic fluid, therapy carried out at a gestational age of 36 weeks or more is often not effective, which is most likely due to severe forms of IUGR and late initiation of treatment. The method of choice for delivery among patients with severe oligohydramnios should be cesarean section.

Thus, the health of the unborn child with oligohydramnios depends on the degree of reduction in the amount of amniotic fluid , the severity of IUGR, the duration of pregnancy, the effectiveness of the therapy and the method of delivery.
