Chymotrypsin is a proteolytic drug of protein origin. When administered intramuscularly, the drug has an anti-inflammatory effect.
When used topically, the drug activates the process of breakdown of necrotic tissues, as well as fibrous type formations (blood clots, blood clots). The drug actively dilutes sputum, exudates, and blood clots.
During exposure to the components of the product, hydrolysis of proteins occurs and the process of breakdown of those compounds that formed amino acids .

Pharmacodynamics
Hydrolyzes proteins and peptones with the formation of relatively low molecular weight peptides, cleaves bonds formed by aromatic amino acid residues (tyrosine, tryptophan, phenylalanine, methionine). Chymotrypsin (EC 3.4.21.1) is synthesized in the pancreas in the form of proenzymes chymotrypsinogen A and chymotrypsinogen B and, in this form, as part of pancreatic juice enters the duodenum, where, under the influence of trypsin, chymotrypsinogens are converted into α-, β- and π- chymotrypsins.
Chymotrypsin preferentially cleaves bonds formed by COOH groups of amino acids having hydrophobic side chains and has a broader specificity of action than trypsin. Chymotrypsin, for example, unlike trypsin, curdles milk.
Chymotrypsin is most active at a pH of 7.5 to 8.2.
Chymotrypsin, trypsin and elastase represent a group of serine proteases due to the presence of serine in their active site. They belong to the same family and make up 44% of the total protein of the exocrine pancreas. According to modern concepts, chymotrypsin and trypsin (similar to the situation with pepsin and gastricsin in the stomach) exist in the form of many isoforms.
A hormone that selectively enhances the secretion of chymotrypsinogen is chymodenine.
Determination of chymotrypsin in feces
Chymotrypsin is one of the most persistent among the proteolytic and lipolytic enzymes of the pancreas, which persists in feces at room temperature for up to two weeks.
Changes in chymotrypsin in feces can indicate disturbances in the secretion of pancreatic enzymes, in particular in patients with chronic pancreatitis. The study is carried out three days after discontinuation of all oral enzyme preparations. It is preferable to take a small amount (1 g) from the daily stool volume. The principle of the method is based on the cleavage of N-acetyl-tyrosine-ethyl ester by chymotrypsin to form acidic products, which are titrated with alkali. In case of severe disorders of the exocrine function of the pancreas, the test detects a significant decrease in the content of chymotrypsin. At the same time, with moderate functional impairments, a fairly significant number of false-positive and false-negative results are observed. In this regard, the determination of fecal chymotrypsin is recognized by most authors as an indicative test for identifying pronounced exocrine disorders of pancreatic function of various nature (Sablin O.A. et al.).

Instructions for use of Chymotrypsin (Method and dosage)
The instructions for use of Chymotrypsin stipulate that initially, before use, it is necessary to dilute the product: for this, 0.005 g of powder should be diluted in 0.5-2% novocaine solution or 1-2 ml of isotonic sodium chloride . For patients with respiratory diseases, Chymotrypsin is administered intramuscularly once a day at a dose of 5-10 mg. Therapy lasts from 12 to 14 days.
For the purpose of prevention and antibacterial treatment, pleural administration of 30 mg once a day is practiced.
Patients with thrombophlebitis need to take 10 mg of the drug intramuscularly for ten days.
When treating inflammatory phenomena and infectious diseases of the sensory organs, 1 ml of Chymotrypsin is instilled onto the mucous membranes with the addition of 1% novocaine twice a day.
The use of this medicine locally is carried out as follows: the product is applied to sterile napkins, after which the napkin must be applied to the site of the lesion or to a purulent wound. In this case, Chymotrypsin (20-40 mg) is used, dissolved in 10 ml of procaine . Therapy continues for ten days.
To prevent complications after operations, daily intramuscular injection of 5-10 mg of the drug is carried out. You need to start treatment 5 days before surgery, after which the medicine is administered for another 3-4 days.
